ISO~Taste of Home forms for free cookware

purchase $25 in P&G products and get free cookbook (from choice of 3) and a rice cooker and tamale steamer cookware set, form needs to be mailed by 5/7, anyone have these? I looked on ebay and cannot find any!

I fund this on Jill Cataldo's site but am also not sure if it's a legitimate form:

I DO NOT KNOW if this is a legitimate posting of the rebate form, but the Smart & Final chain of stores has the P&G mail-in offer form posted on their website:

It's the blue link at the top of the page. Of course, the form itself says it cannot be reposted on a website without permission from P&G. But at least you'll know what the form looks like... or try it at your own risk.

Thanks Kelly!! Just yesterday I was cleaning up and threw away a rct from Kmart and our covergirl purchases- luckily I hadn't taken the trash out and found the rcpt- unscathed in the trash can!!!
Woo Hoo- not that I know how to cook Tamales, but a Rice Cooker will be nice!
It's worth a stamp to give it a shot!

Guess what- this came today!!! Rice Cooker, Tamale steamer, but no cook book, and no letter as to what it was from. I really had forgotten about this! I had to rack my brain to figure out where it came from.
